With our life insurance options, you can give your employees basic life
insurance, plus the opportunity to buy more coverage for themselves
and their families through payroll deduction.
Note to Participating Members: All AMHIC groups do not offer all AMHIC plans. Please check with your human resources department to confirm which AMHIC plans are offered by your employer.
Basic Life Insurance and AD&D
With the MetLife Basic Life Insurance & Accidental Death and Dismemberment (AD&D) Plan, you can choose a plan that fits your organization's budget: a flat $50k per employee, 1 times, 1.5 times, or 2 times base annual earnings to a maximum of $500,000 (not combined with optional life insurance).
Optional Employee Life Insurance
You can offer MetLife Optional Employee Life Insurance as an added benefit for employees who want to buy more coverage through payroll deduction. They can purchase 1, 2, 3 or 4 times their base annual earnings to a maximum of $500,000, not combined with basic life insurance. For coverage amounts exceeding 3 times base annual salary, or $100,000 (whichever is the lesser amount), they'll need to complete a Statement of Health Form, and be approved.
Dependent Life Insurance
With Dependent Life Insurance, you can help your employees buy coverage for their spouse, domestic partner or children. Through payroll deduction, employees can purchase between $10,000 and $100,000 for a spouse or domestic partner, or $5,000 to $25,000 for dependent children. For coverage exceeding $10,000, they'll need to complete a Statement of Health Form, and be approved.
